Patient Safety – Diagnostic and Medication Safety

This course focuses on improving patient safety by addressing two critical areas in healthcare delivery: diagnostic accuracy and safe medication practices. Participants will learn how diagnostic errors and medication mishaps contribute to patient harm, and how to implement preventive measures in line with Ghana Health Service protocols and global best practices.

Through interactive sessions, real-life case studies, and role-plays, healthcare professionals will explore root causes of diagnostic and medication errors, including communication breakdowns, human factors, system failures, and poor documentation. The course will also emphasize strategies for medication reconciliation, accurate documentation, patient education, and collaborative decision-making to enhance safety and reduce preventable harm.
